Understanding Neuroplastic Pain: How Psychotherapy Can Help Relieve Chronic Pain

Chronic pain is often thought of as a sign of physical injury or tissue damage. However, growing research suggests that not all pain originates from ongoing structural problems. In some cases, pain can be driven by changes in the nervous system itself. This type of pain is often referred to as neuroplastic pain.

Understanding neuroplastic pain can be life-changing for individuals who have struggled with persistent symptoms despite medical treatments, surgeries, or normal imaging results. Fortunately, psychotherapy offers evidence-based tools to address the brain-based mechanisms that contribute to chronic pain and support long-term recovery.

What Is Neuroplastic Pain?

Neuroplastic pain occurs when the brain and nervous system become sensitized and continue generating pain signals even after an injury has healed or when no significant tissue damage is present. The term "neuroplastic" refers to the brain's ability to adapt and change throughout life.

Research suggests that chronic pain can become a learned neural response. Over time, the brain may begin interpreting normal sensations as threatening, producing real pain even in the absence of ongoing injury. Neuroplastic pain does not mean the pain is imaginary—it is very real. Rather, the source of the pain lies primarily in altered neural pathways and nervous system processing.

Common conditions associated with neuroplastic mechanisms include chronic low back pain, fibromyalgia, irritable bowel syndrome, tension headaches, temporomandibular joint (TMJ) disorders, and certain forms of chronic pelvic and neck pain.

The Connection Between Stress, Emotions, and Pain

Pain is not solely a physical experience. The brain continuously evaluates information from the body and environment to determine whether a sensation signals danger. Stress, anxiety, trauma, and emotional distress can increase the brain's perception of threat and amplify pain signals.

When the nervous system remains in a state of heightened alertness, it becomes increasingly sensitive to normal bodily sensations. This can create a self-reinforcing cycle in which pain increases fear, fear increases nervous system activation, and nervous system activation further intensifies pain.

Breaking this cycle is often a central goal of treatment.

How Psychotherapy Can Help Neuroplastic Pain

Psychotherapy can be highly effective for neuroplastic pain because it targets the cognitive, emotional, and neurological processes that contribute to chronic symptoms.

Pain Education

Learning about the neuroscience of chronic pain can reduce fear and uncertainty. Understanding that pain does not always indicate physical damage helps many individuals feel safer in their bodies.

Cognitive Restructuring

Therapy can help identify and challenge catastrophic thoughts and beliefs about pain. Reframing pain-related fears can reduce the brain's threat response and decrease symptom intensity.

Emotional Processing

Chronic stress, unresolved emotions, and past traumatic experiences can contribute to nervous system sensitization. Therapy provides a safe environment to process these experiences and develop healthier coping strategies.

Mindfulness and Somatic Awareness

Many therapeutic approaches teach individuals to observe physical sensations with curiosity rather than fear. This shift in attention can help retrain the brain to interpret bodily sensations as safe rather than threatening.

Nervous System Regulation

Psychotherapy often incorporates relaxation techniques, breathing exercises, and stress-management skills that calm the nervous system and promote recovery.

The Evidence Supporting Psychological Treatment for Chronic Pain

A growing body of peer-reviewed research supports the role of psychological interventions in treating chronic pain.  The following types of therapy are well-established chronic pain interventions:

  • Cognitive Behavioral Therapy (CBT):  In CBT, patients focus on changing uphelpful thought patterns, lowering hyerparousal and modifying behaviors to improve functioning. CBT is well established and effective. Recent meta-analysis revealed moderate to large effect sizes for pain intensity, catastrophizing, interference,  depression and QOL treatment improvements (Murphy et al., 2023).

  • Acceptance and Commitment Therapy (ACT):  With ACT patients learn to shift the focus from fighting or trying to eliminate pain to building psychological flexibility, practicing acceptance and engaging in meaningful daily activities. Studies indicate that effectiveness of ACT is comparable to other psychological pain treatments with improvements in pain expensive, QOL, pain functioning and lowered depression and anxiety (Ma et al., 2023).

  • Pain Reprocessing Therapy (PRT): In PRT patients retrain their brain to unlearn pain, specifically targeting neuroplastic pain where the nervous system treats safe body signals as a danger. It uses education, somatic tracking, and emotional regulation to stop the pain cycle. This newer treatment is very encouraging with 66% of patients who completed PRT reported being pain free and with maintained benefits at a year (Ashar et al., 2022).

  • Emotional Awareness and Experience Therapy (EAET): EAET helps chronic pain by targeting the brain and processing hidden, stressful or avoided emotions. It uses education, trauma disclosure, and emotional expression to reduce centralized pain symptoms. Similarly this treatment is found to reliably reduce pain and pain interference in life (Lumley & Schubiner et al., 2020).

  • Empowered Relief: In this single session, 2 hour intervention individuals learn neuroscience pain basics, fast coping skills and the use of mindfulness. It has been found to cut down stress, improves sleep and reduce pain with improvements comparable to longer courses of CBT (Darnell et al., 2024).
